<p class="wp-block-paragraph">Paciente masculino, 45 anos, previamente hígido, foi submetido a colonoscopia em outro serviço que identificou um pólipo séssil de 6 mm de diâmetro, 0-Is pela classificação de Paris, com superfície lisa e amarelada, localizado em reto médio. Na ocasião foi realizada ressecção parcial da lesão com alça a frio. Resultado anatomopatológico e imunohistoquímico evidenciaram tumor neuroendócrino bem diferenciado (grau 1 – Ki67&lt;2%), com margens comprometidas. </p>



<p class="wp-block-paragraph">Paciente veio encaminhado para realizar nova colonoscopia na tentativa de ressecção completa da lesão. Durante o procedimento foi observada uma diminuta lesão amarelada no reto, discretamente elevada, correspondente à área de polipectomia prévia com presença de lesão residual (Figuras 1, 2 e 3). Realizada tentativa de mucosectomia pela técnica de imersão (“underwater”), não havendo pega adequada com a alça para ressecção. Foi optado, então, pela realização da mucosectomia por imersão assistida por cap, que consiste na imersão do espaço intraluminal com água, seguido por sucção da lesão com auxílio de cap endoscópico, afim de formar um pseudopólipo, e assim facilitar a apreensão e ressecção da lesão (Figura 4). Com o uso dessa técnica foi possível apreender a lesão residual com a alça e realizar sua ressecção completa (Figuras 5 e 6). O resultado anatomopatológico confirmou a presença de tumor neuroendócrino bem diferenciado (grau 1 – Ki67&lt;2%), com margens laterais e profunda livres.</p>

<h2 class="wp-block-heading"><strong>Discussão</strong></h2>



<p class="wp-block-paragraph">A mucosectomia underwater assistida por cap (CAP-UEMR) consiste na utilização de cap endoscópico para sucção da lesão a ser ressecada sob imersão em água, até que seja formado um “pseudopólipo” passível de apreensão e ressecção. Se a ressecção em monobloco não for possível, pode-se realizar novos “pseudopólipos” e ressecar à piece-meal, até que se alcance o resultado desejado, conforme ilustrado na figura abaixo:</p>

<p class="wp-block-paragraph">O estudo foi uma análise observacional retrospectiva de 83 procedimentos de ressecção endoscópica pela técnica CAP-UEMR, realizados em dois centros entre setembro de 2020 e dezembro de 2021. O desfecho primário foi o sucesso técnico, definido como ressecção completa macroscópica da lesão no índice CAP-UEMR. Os desfechos secundários foram as taxas de sangramento e perfuração. As 83 lesões tratadas tinham um tamanho médio de 20 mm. Foram incluídas 64 lesões deprimidas ou planas (18 previamente manipuladas, 9 com acesso difícil), 11 lesões do apêndice e 8 lesões da válvula ileocecal. Os resultados mostraram uma taxa de sucesso técnico de 100%, com ressecção macroscópica completa alcançada em todas as 83 lesões. Houve 7 casos de sangramento intraoperatório e 2 casos de sangramento tardio, todos tratados endoscopicamente. Nenhuma perfuração ou outras complicações ocorreram. Entre as 64 lesões com colonoscopia de acompanhamento, apenas 1 recorrência foi detectada, que foi tratada endoscopicamente.</p>



<p class="has-pale-ocean-gradient-background has-background wp-block-paragraph">Concluiu-se que a CAP-UEMR pode ser uma técnica segura e eficaz para facilitar a ressecção de lesões colorretais complexas. O estudo possui suas limitações, sendo as principais o possível viés de seleção e design retrospectivo e necessidade de estudos comparativos para determinar a eficácia específica do CAP-UEMR em relação a outras técnicas de ressecção.</p>
